Parental Consent Letter
For children and teenagers under 18 years old, if a parent is not joining the mission trip, a parental consent letter must be signed.
Filling and signing the following letter as a parent (or legal guardian) provides your legal signature and states that you and your child have completely reviewed the information provided by the Rice Foundation regarding the terms of service, waiver of responsibility, and policy for volunteer service.
